Has anyone had this problem (plus solution )

Sometimes when ppl enter my server, theyre name dissapears, so theyre nick is then ( ), how does this happen, and how do i solve this problem, its pretty anoying..

Yeah ive seen this happent on more then one server but it did`n effect all ppl..but i dunno what the solution to fix this is
